Postpartum care, connected

A referral isn't care until care happens.

AfterBirth helps maternity care teams manage postpartum mental health referrals, coordinate follow-ups and keep the next step in care visible—so fewer mothers are lost between screening and support.

The appointment ends. The need for care doesn't.

For many mothers, mental health concerns don't disappear after the postpartum visit. Some symptoms emerge later. Others require ongoing assessment, referrals and support.

When follow-up depends on disconnected processes, busy teams can struggle to see whether a patient has taken the next step.

Mothers deserve continuity. Care teams deserve the tools to deliver it.

No mother should have to navigate the gap between referral and care alone.

The operational gap

Your team shouldn't have to chase care across disconnected workflows.

The challenges AfterBirth CareOS is being designed to address. Every practice is different—these are the patterns we're investigating with care teams.

  1. 01

    Referrals without visibility

A referral is made, but its next status may not be readily visible to the referring team.

  2. 02

    Follow-ups that require manual coordination

Staff may rely on calls, notes, spreadsheets or separate systems to track outstanding actions.

  3. 03

    Limited visibility into care completion

Knowing that a referral was sent isn't the same as knowing whether an appointment occurred.

  4. 04

More work for already busy teams

Care coordination can add administrative pressure to clinical workflows.

One workspace. Clearer next steps.

From referral to follow-up, see the whole journey.

AfterBirth CareOS is being designed to help maternity care teams organize postpartum mental health referrals, assign follow-up tasks and maintain visibility into each patient's next step.

How it works

  1. Step 01

    Record the referral

Document the next step recommended by the care team.

  2. Step 02

    Coordinate the follow-up

Assign responsibilities, manage tasks and keep outstanding actions visible.

  3. Step 03

    Track progress

Record appointment booking, attendance confirmation and unresolved referral statuses.

  4. Step 04

    Close the loop

Document the outcome and help the team identify whether additional follow-up is needed.

One workflow designed around what happens after the referral.

Built for the people coordinating care.

For practice administrators

A clearer view of outstanding work, referral status and operational performance.

For care coordinators

An organized workspace for assigned tasks, follow-up schedules and referral progress.

For clinicians

Better visibility into documented next steps without replacing clinical judgment.

Why postpartum mental health

Mental healthcare shouldn't stop at the six-week visit.

Postpartum mental health conditions, including depression and anxiety, can emerge at any point during the first year after childbirth—not only in the weeks surrounding delivery.

ACOG recommends that postpartum care be an ongoing process rather than a single encounter, and that screening be paired with systems that connect patients to appropriate assessment, treatment and follow-up.

Screening identifies a need. Coordination determines whether that need is met.

Help shape the next chapter of postpartum care.

We're inviting US maternity care providers to help us understand current referral workflows, identify operational challenges and shape the first version of AfterBirth CareOS.

Design partners will have an opportunity to review early product concepts and provide feedback on the workflows that matter most to their teams.

Become a Clinical Design Partner

Participation in product discovery does not involve patient care or live clinical monitoring.

Because what happens after the referral matters.

We're building AfterBirth around a simple belief: the path from identifying a mental health concern to receiving appropriate care should be easier to coordinate, understand and navigate.
